When reassigning individual icons, avoid selecting generic icons by number (e.g., "icon #11"). Instead, click Browse and select a uniquely-named, physical .ico file. This prevents Windows from renumbering the package and causing icons to get "out of whack".

Initially, I was worried. Windows 11’s new File Explorer and centered taskbar broke many legacy customizers. But Stardock pushed an update in early 2023 that works natively with Windows 11 22H2 and 23H2. Even the new context menu icons can be changed (though you need to toggle "Advanced Replacements" in settings).
