The film starts with a flashback to 1817, where a group of coal miners, led by the ruthless and cunning A.J. (played by Andrew Prine), stumble upon the isolated home of a family with a dark secret. The family's matriarch, Mother, possesses a taste for human flesh, and A.J.'s men soon find themselves becoming her next meal. Horrified by what they've witnessed and fearing for their own lives, A.J. and one of his miners make a brutal attempt to eliminate the family. However, they fail to eradicate the clan's lineage, setting the stage for the events of the previous films.

Deke Richards brings a fresh perspective to the series, perhaps more focused on gore and less on the atmospheric tension that made the first Wrong Turn stand out. The film opens in 1974 at a remote West Virginia sanatorium for the criminally insane, the Glensville Hospital. Two mutant brothers, referred to as "Three-Finger" and "One-Eye," are patients (alongside a third, Saw-Tooth, elsewhere). When a massive blizzard causes a power outage, the brothers escape, killing staff and patients alike. They’re eventually cornered in the boiler room, and the head doctor locks them in to freeze to death.
